Plain-language legal information for ULC patients and visitors.

How information is used

ULC collects only the information needed to respond to enquiries, manage bookings, prepare appointments, record procedure acknowledgements, keep patient administration records and coordinate care. Health information and signature records are handled as special personal information under POPIA.

  • Information collected may include contact and identification details, account holder and medical aid details, next-of-kin details, referral information, relevant health history, medicines, allergies, consent acknowledgements and electronic signatures.
  • Information is used for practice administration, appointment communication, clinical preparation, care coordination, account administration, consent records and legal record-keeping.
  • If you opt in to save an registration draft, it remains in this browser for up to seven days. Drawn or typed signatures are excluded from local drafts. Avoid this option on a shared device.
  • This website does not store submissions. Completed forms are transmitted directly to the practice inbox, where the practice stores and manages them in its own records. Information is not placed in URLs or sent to advertising services.
  • Information is retained for the period required by South African law and practice policy. [CONFIRM: retention period and Information Officer].
  • Access, correction, objection or deletion requests, where legally applicable, can be sent to info@urogynaecology.co.za.
  • The practice must maintain appropriate written operator arrangements with service providers that process personal information.
