These terms describe how this website, appointment requests, digital registration and consent records may be used.
Website and digital forms
The ULC website provides practice information, educational clinical guides and administrative pathways. Website use or a submitted form does not by itself create a practitioner-patient relationship.
- Appointment requests are reviewed and are not confirmed until the practice confirms them directly.
- Digital registration supports administration. It is not a clinical assessment, diagnosis or procedure consent.
- Digital procedure consent records acknowledgements after a clinical discussion. Patients may withdraw or ask further questions before the procedure, subject to clinical and legal requirements.
- Medical aid, authorisation, payment and cancellation wording must be confirmed by the practice before publication. [CONFIRM]
- Do not use website forms for emergencies. Seek urgent care when symptoms are severe or sudden.
